Landlords must employ this form, or a form of their own that contains all of the prescribed data, at the beginning of a tenancy.

A landlord must complete this form and provide all required data if they are applying for a rehabilitation scheme.

Application for Approval of Rehabilitation Scheme for Specified Unit (Course 5B) (legal size)
A landlord must complete this form and provide all required information if they are applying for a rehabilitation scheme for a specified unit.

Notice to Tenant – Rehabilitation Scheme for All or Part of a Residential Complex (Form half-dozen)

A landlord must use this course, or a form of their own that contains all of the prescribed data, to notify tenants of tenant services charge increases.

This letter can exist given to tenants who have received a notice of termination for non-payment of rent.
